Access Support Coordination (ASC) are specialists in providing their clients with the right access to supports to accelerate their independence and achieve their goals.

About this opportunity:

The Support Coordinator is responsible for connecting participants with a NDIS Plan to formal and informal services and supports in their community and assisting participants in understanding and reviewing their NDIS plan.

Key responsibilities of this role:
  • Build the capacity of the participants to achieve greater independence to self-direct services and support in the long term
  • Ensure mainstream services meet their obligations and organisational responsibility
  • Provide the participants and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
  • Oversee the implementation of service agreements for participants including funding capacity
  • Assist participants to understand service options and to make decisions in relation to preferred options
  • Link participants to service providers, and support participants to obtain quotes and establish service agreements with selected providers
  • Ensure evidence is collated to improve client outcomes
  • Develop relationships with key community stakeholders & liaise with other statutory bodies and government departments
  • Investigate suitable options for service users in the way of recreational, career orientation or accommodation options
To succeed in this role you will have:
  • Experience in a Support Coordination role or similar for a minimum of 2 years (specifically with mental health is highly desirable)
  • Certificate IV in Disability or other relevant qualification (highly desirable)
  • Strong knowledge of local disability community and providers
  • An understanding of billable hours in relation to KPI targets
  • Basic accounting and budgeting skills
  • Excellent written and verbal communication skills
  • Working with Children Blue Card and NDIS Worker Screening/Yellow Card
  • Drivers Licence
