Our partner Access Support Coordination (ASC) are specialists in providing their clients with the right access to supports to accelerate their independence and achieve their goals. ASC currently have an opportunity in Gympie!

At ASC, we engage only the best and most experienced Support Coordinators that have a broad variety of skills, knowledge and understanding of the disability and mental health sector.

This is a permanent part time role - 3 days / 24 hours per week.

Your responsibilities:
  • Ensure mainstream services meet their obligations and organisational responsibility
  • Build the capacity of the participants to achieve greater independence to self-direct services and support in the long term
  • Provide the participants and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
  • Oversee the implementation of service agreements for participants including funding capacity
  • Assist participants to understand service options and to make decisions in relation to preferred options
  • Link participants to service providers, and support participants to establish service agreements with selected providers
  • Ensure evidence is collated to improve client outcomes
  • Identifying the services that meet the participants needs, maximising participants supports, enabling participants to feel in control of their supports and coordinating service providers, liaising with government agencies, obtaining quotes for services, and implementing supports required to achieve their goals
  • Developing relationships with key community stakeholders
  • Liaising with other statutory bodies and government departments
  • Investigating suitable options for service users in the way of recreational, career orientation or accommodation options
Key skills and about you:
  • A combination of relevant experience, education and training in a similar role for a minimum of 2 years
  • Strong knowledge of local disability community and providers
  • Basic accounting and budgeting skills
  • Excellent written and verbal communication skills
  • Experience in mental health/PHaMs/PIR
Qualifications & Licences:
  • Drivers Licence
  • Suitability Blue Card for Working with Children and Young People
  • Yellow Card or NDIS Worker Screening Check
  • Certificate IV in Disability or other relevant qualification (desirable)
